Registration

I. ACKNOWLEDGMENT OF RISK
I, the undersigned parent or legal guardian, understand that my child’s participation in the Capital City Young Guns Summer Program involves the use of firearms and live ammunition. I acknowledge that shooting sports carry inherent risks, including but not limited to serious injury or death. I certify that my child is physically and mentally fit to participate.

II. RELEASE OF LIABILITY
In consideration for allowing my child to participate in the program, I hereby release and discharge Capital City Young Guns, its officers, board members, volunteers, sponsors, and property owners from any and all claims, liabilities, demands, or causes of action arising out of or related to any injury, loss, or damage that may occur during participation.

III. MEDICAL CONSENT
In the event of an emergency, I authorize program staff or volunteers to secure medical care and treatment for my child. I understand that reasonable efforts will be made to contact me before such action is taken. I accept responsibility for any medical expenses incurred.

IV. PHOTO & MEDIA RELEASE
I grant Capital City Young Guns permission to use photos or video footage of my child taken during the program for promotional, educational, or fundraising purposes in print or online media.

V. CODE OF CONDUCT & ATTENDANCE POLICY
I understand and agree to the following program expectations:

My child will follow all safety rules and directions from instructors and volunteers.

My child is allowed 2 unexcused no-shows/no-calls during the season. Additional unexcused absences will result in dismissal from the program.

Students who complete the full season may receive participation rewards.

VI. INDEMNIFICATION
I agree to indemnify and hold harmless Capital City Young Guns from any liability, including attorney’s fees, resulting from my child's actions during the course of the program.

By signing below, I acknowledge that I have read and understood this waiver and agree to its terms.
